Across Cancer Care Alberta (CCA), Supportive Care services are delivered by a dedicated team of health care professionals committed to improving the quality of life for individuals affected by cancer. This support begins at diagnosis and continues through treatment and beyond. CCA's team is a recognized leader in cancer prevention, diagnosis, treatment, survivorship, and palliative care. We are looking for a dedicated, patient-centered and team-oriented Social Worker (SWII) to join our team at the Margery E. Yiull Cancer Centre in Medicine Hat. In this permanent part-time role, you will be a key member of the local and provincial teams in supporting patients affected by cancer, helping them navigate the social determinants of health. Key Responsibilities include: Referral triage, screening, assessment, resource and supportive counseling. Working collaboratively with a team of healthcare professionals to enhance the patient experience. Developing and facilitating psychoeducational groups with other healthcare professionals. Screening for distress and high needs to identify and remove barriers to cancer treatment. Participating in quality improvement initiatives, research, and service evaluation. Engaging in continuous professional development and education. Supporting orientation, supervision and mentorship of new employees and students’ supervision. Connecting patients to community programs like Wellspring Alberta. Caseloads for all positions within Supportive Care may change depending on operational needs. Currently, the days of work are Monday to Friday for this position, no weekends. This role may also provide virtual or in-person coverage at the Jack Ady Cancer Centre as required. Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. Within the last two years, clinical work and/or practicum in oncology or complex health care settings, with demonstrated ability to provide psychosocial assessment, counseling, advocacy, and supportive care services to patients and families affected by cancer. Within the last two years, experience facilitating groups. Experience providing virtual care. Understanding of trauma-informed practice principles, including safety, trust, choice, collaboration, empowerment, and cultural humility, with the ability to integrate these principles into all aspects of patient care and interdisciplinary team engagement. Demonstrated knowledge of the emotional, social, financial, and practical impacts of cancer diagnosis, treatment, survivorship, and palliative care. In-depth knowledge of advanced care planning, income security programs, housing, and support services to anticipate patient and family needs. Experience providing supportive counseling, grief, and loss support. Demonstrated skills in assessment, intervention, system navigation and case management. Effective time management, self-direction and initiative are required. Excellent communication skills.
Preferred Qualifications:
Completion of Connect Care Training (ILTs) for Allied Health. Proven ability to establish, foster, and sustain collaborative partnerships across multidisciplinary teams and external organizations. Evidence of ongoing professional development in psychosocial oncology, suicide prevention, or crisis intervention. Previous experience working within Cancer Care Alberta or a strong understanding of oncology care pathways and services. Advanced proficiency in Excel, PowerPoint, Word, and Outlook.
