ABOUT THE HOSPICE OF WINDSOR AND ESSEX COUNTY
Founded to provide compassionate, clinical, end-of-life care, The Hospice of Windsor and Essex County Inc. (Hospice) has been caring for our community since 1979. Hospice believes in equitable access to care for all. We provide specialized wrap-around services to adults, children, and their caregivers, in the setting of their choice – whether it be in our Residences or in their home.
With only partial government funding, it has been through the generous support of our volunteers and our community that we have continued to provide memorable care, meeting the physical, emotional, and spiritual needs of patients and their loved ones.
ROLE OVERVIEW
The Social Worker is dedicated to Hospice’s vision, mission, and values, and will support and work towards achieving the strategic directions set out by the Board of Directors. They will put their best efforts forward to promote the interests of our patients as well as contribute to a positive work culture.
The Social Worker is experienced and knowledgeable about life-altering and life-threatening diagnoses and the broader field of palliative care, with special emphasis on end-of-life care. The Social Worker manages a caseload of patients with life altering/threatening diagnosis and their caregivers as well as running psychosocial group programs, demonstrating professional values and compassion for patients, families and visitors. They develop and promote relationships with community health partners.
PRIMARY DUTIES AND RESPONSIBILITIES
- Receive referrals from the Manager of Psychosocial Services or Clinical Administrative Assistants and respond to clients within established response guidelines.
- Provide individual and family counselling, as well as the development and facilitation of group programs.
- Complete psychosocial assessments, develop goals and related plans of care in collaboration with the client/family, and make recommendations for appropriate social work intervention.
- As part of an Interdisciplinary team, design and implement group counselling and programs that address common psychosocial needs of patients and caregivers.
- Communicate, implement and ensure that Hospice’s responsive and holistic culture is maintained.
- Develop and nurture professional/therapeutic relationships with community partners.
- Communicate and implement Hospice’s support services as appropriate for clients/families.
- Assess psychosocial education needs and assist in providing staff and volunteer education.
- Participate in team meetings for case consultation and information sharing.
- Complete electronic scheduling and documentation for assigned patients and complete ongoing updates and monthly statistical reports, while prioritizing patient needs.
- Attend meetings and conferences for coordinating services and/or professional development.
- Assist with the supervision of University Social Work interns and students in related programs, as deemed beneficial to the progression, promotion, and development, of Hospice support services and community palliative care.
- Participate in employee engagement initiatives and provide feedback to contribute to program development and quality improvement projects.

ACCOUNTABILITIES
- Accountable to the Manager of Psychosocial Services. Case reviews are completed with the Manager of Psychosocial Services for ongoing performance evaluation.
- Accountable for commitment to the Hospice Mission, Vision and Values.
- Accountable to maintain all Hospice policies and professional standards of conduct, and to ensure review as required.
- Accountable to maintain confidentiality and act in an ethical and professional manner consistent with the professional code of ethics and agency policies.
- Accountable to maintain practice requirements through the annual renewal of the professional college certification and to exhibit clinical competence through continuing education and established protocols as required by the Ontario College of Social Workers.
- Accountable to maintain performance in accordance with the palliative care competencies for social workers as defined by the Ontario Palliative Care Competency Framework (Reference: The Ontario Palliative Care Competency Framework: A Reference Guide for Health Professionals and Volunteers, April 2019, pp. 126-137).
- Accountable for adherence to HPCO and CARF accreditation standards.
WHAT WE OFFER
- Hourly pay: $37.91 - $42.31 per hour full-time.

- Paid Time Off: Employees receive a minimum of three (3) weeks of paid vacation annually.
REQUIRED QUALIFICATIONS AND EXPERIENCE
- TECHNICAL REQUIREMENTS
- Legally entitled to work in Canada.
- Valid Police Record Check including vulnerable sector check (within six months of hire, and declaration of offences as required).
- Maintains current registration in good standing with the Ontario College of Social Workers and Social Service Workers and provides proof of annual renewal by December 31 of each year.
- Demonstrates ability to perform as an individual counsellor and to provide group development and program facilitation.
- Demonstrates excellent oral, written and telephone communication skills.
- Experience with electronic documentation systems, spreadsheets, and/or database systems is preferred.
- Possesses a valid driver’s license and reliable vehicle.
- Works a flexible schedule as required.
- Demonstrates ability to work in a collaborative team environment.
- French language skills are an asset.
- EXPERIENCE
- Experience with grief and bereavement through all patient populations is preferred.
- Possesses five (5) years of work experience in a social work role, preferably in palliative or healthcare settings.
- EDUCATION
- Possess a Master of Social Work Degree from a recognized university program.
