Description

The Unit Manager is responsible for the operational leadership, management, and coordination of assigned clinical units and services within the hospital. As a key member of the leadership team, the Unit Manager ensures the delivery of safe, high-quality, patient-centred care aligned with the organization’s mission, values, and strategic priorities.

Working at the middle-management level, the Unit Manager provides leadership to interdisciplinary healthcare teams and oversees the effective utilization of human, financial, and physical resources to support clinical excellence and operational efficiency.

The Unit Manager is accountable for achieving established unit and organizational Key Performance Indicators (KPIs) including quality, safety, patient experience, workforce engagement, and fiscal performance. They ensure their teams deliver extraordinary patient care tailored to the needs of the clinical portfolio, while fostering a culture of collaboration, accountability, innovation, and continuous improvement.

Portfolio-specific operational deliverables will be identified and must be achieved in alignment with established performance metrics and organizational priorities.

Unit Managers are assigned portfolios based on operational requirements. Periodically portfolios/unit assignments are adjusted to meet operational and patient care needs and to support career growth opportunities for leadership team members.

Assignment Overview:

The Unit Manager - Acute Care is responsible for the overall leadership, management, and operational performance of a specialized inpatient unit that supports patients transitioning from acute care to home, community, or long-term care settings. The role works collaboratively with an interdisciplinary team and ensures the delivery of high-quality, patient-centred, and evidence-informed care aligned with organizational priorities, Ontario Health standards, and applicable legislation.

The role is accountable for staffing, resource utilization, quality and safety outcomes, and the effective coordination of care transitions in partnership with internal departments and external stakeholders, including community agencies and long-term care providers.

The Unit Manager ensures that care delivery within the 2E unit promotes functional restoration, discharge readiness, and optimal patient flow across the hospital. This includes overseeing care planning processes, supporting timely and appropriate discharges, and addressing system barriers that impact length of stay and alternate level of care (ALC) pressures. The role actively contributes to strategic initiatives, quality improvement activities, and organizational change efforts aimed at enhancing patient outcomes and system integration.

Through visible leadership and engagement, the Unit Manager promotes a safe, respectful, and inclusive environment for patients, families, staff, and partners, with a strong emphasis on patient and family-centred care and equitable access to services across diverse populations, including Indigenous communities.

Key Project:

This role will actively participate in the implementation of a Hospital to Home (H2H) care program and shape the future of the H2H program delivery. The goal of the program, launching at hospitals around Ontario, is to expand access to care at home for patients who no longer require acute care in a hospital setting, while also reducing avoidable return visits to the Emergency Department and hospital readmissions.
