The Transitional Housing Program Case Manager works as part of the Housing Services Team that provides programs and services for individuals with affordable/social housing needs including Monthly Housing for adult men and women, Transitional Housing for adults and youth, as well as counselling and referrals. The Case Manager will provide direct service to clients (males or females) referred/housed in the Transitional Housing Program and work with the Program Director and Housing Services team in providing excellent service delivery.
The hours for this position are 11am to 7pm from Tuesday to Saturday.

The Transitional Housing Program Case Manager works as part of the Housing Services Team that provides programs and services for individuals with affordable/social housing needs including Monthly Housing for adult men and women, Transitional Housing for adults and youth, as well as counselling and referrals. The Case Manager will provide direct service to clients (males or females) referred/housed in the Transitional Housing Program and work with the Program Director and Housing Services team in providing excellent service delivery.

Some Of The Tasks Of This Position Include
- Providing information to individuals and agencies about the Transitional Housing Program and general information regarding the Y’s Housing Services.
- Taking referral information and conducting intake interviews with program referrals.
- Developing and maintaining individual goal plans with assigned male Transitional Housing Program clients.
- Providing case management and counselling support, crisis intervention and referrals for assigned clients.
- Providing life and social skills workshops.
- Maintaining standards for reporting and operating procedures.
- Reporting statistical data.
- Assisting with the organization and coordination of special events.
- Ensuring client confidentiality and that all files and records are secure.

The Successful Candidate Will Have The Following
Education
- Post-secondary education or equivalent in a related field
- Fluent in English and French, oral and written
- Other languages are an asset
- Minimum 2 years’ experience in Social Services, preferably in the non-profit sector
- Knowledge and sensitivity in dealing with women and individuals with issues concerning homelessness, addiction, mental health, gender and sexual orientation
- Ability to work with clients with diverse backgrounds and needs
- Excellent knowledge of community resources
- Facilitation skills for workshops and information sessions
- Proficient in MS Office Suite
- Strong communication skills
- Strong problem-solving and creative skills
- Applicants must have a current Standard First Aid and CPR “C” certification.
- Applicants must already have a valid permit to work in Canada.
- A Police Record check, not more than 6 months old is required. Police Records checks are reviewed on an individual basis and the offense(s) - if any - is considered in the decision-making process in relation to the requirement of working with children, therefore not eliminating all candidates with a record from being offered a position.
- Semi-annually, staff must complete the mandatory training for the protection of Children & Youth and submit the certificate of completion.
