We are GROWING and have one new position available to expand one of our Primary Care Action Teams. Thames Valley Family Health Team is looking for a Nurse Practitioner (MRP). This position will initially start at 4-141 Dundas Street West, Thamesford, Ontario.
The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive collaborative primary health care to over 169,000 patients.

We are creating a new model of care and are looking for providers that want to shape the future. The Nurse Practitioner (NP - MRP) will be the primary care provider to an individual panel of patients and function as a member of the broader primary care team providing accessible and essential primary health care services in the Thames Valley Family Health Team (TVFHT). The NP provides services to patients who include the individual, the family, and the community, throughout their life span with a strong emphasis on health promotion, patient education and disease prevention.

Requirements
- Current registration with the College of Nurses of Ontario as a Nurse Practitioner is required.
- Work to full scope including ability to prescribe controlled substances as authorized by CNO without restrictions
- 2 years experience as a Nurse Practitioner preferred
- Council of Ontario University Program in Nursing Primary Health Care Nurse Practitioner Program Certificate (or recognized equivalent)
- Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred
- Current Membership with RNAO or NPAO and evidence of professional liability insurance
- A solid understanding of the roles and responsibilities of MRP (most responsible practitioner)
- Demonstrated experience in Community and/or Primary Care Setting
- Availability to work extended hours, including after-hours clinic
- Be available for follow up of critical value lab results after hours by telephone
- Work is completed on site; this is not a hybrid work position
- Have reliable transportation to allow travel between sites as needed
- Practice at full scope with Most Responsible Provider (MRP)
- Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
- Strong leadership, organization, research, evaluation, time management and interpersonal skills
- Experience in the use of EMR, Microsoft Word, Excel and Outlook.
- Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
- Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient
- Excellent verbal and written communication skills
- Exhibit an ability to be open and non-judgmental
- Knowledge of the goals and structure of the organization
- Experience and a commitment to leverage other Interprofessional Healthcare Providers (IHP) in the provision of care to panel of patients.
