Your impact

In this role, you will:
- Develop personalized, evidence-informed care plans to support individual rehabilitation and wellness goals.
- Collaborate with a multidisciplinary team to coordinate patient-centric care and achieve optimal outcomes.
- Build the next generation through mentorship and leadership (as assigned).
- Stay current with best practices through continuous learning and professional development.
- Utilize modern tools, including e-charting and digital communication, to streamline your workflow.
- Partner with therapy assistants and kinesiologists to maximize clinical impact.
- Educate patients and their families on recovery, prevention, and self-management strategies.

Above all, you are driven to grow – both professionally and personally. Key skills and qualifications include:
- Active license in good standing with your provincial regulatory college.
- Professional liability insurance (as required).
- Exceptional interpersonal skills and a patient-first mindset.
- Enthusiasm for mentoring, participating in clinic events, and sharing knowledge.
- 1–2+ years of outpatient LTC experience (preferred; new grads with provisional licenses are encouraged to apply!).
- Ability to thrive in a collaborative, team-oriented environment.
- Assistive Devices Program (ADP) authorization mandatory
- A valid Vulnerable Sector screening check (within 6 months)
- Proof of Tuberculin (2-Step Mantoux Test) clearance
