Job Description
On behalf of our Healthcare client, Affinity Healthcare Solutions is looking for a to support a major organizational transition across business, operational, technical, and workforce workstreams. This role will provide hands-on planning, integrated scheduling, RAID management, stakeholder coordination, governance support, reporting, and transition readiness activities for an initial six-month engagement, requiring approximately 2–3 days per week.

Responsibilities
Project Planning and Coordination
- Develop and maintain a project charter or Project Definition Document (PDD-Lite).
- Define scope, assumptions, constraints, dependencies, risks, and change management considerations.
- Establish and coordinate IMIT working groups and stakeholder engagement activities.
- Develop and maintain project work plans and implementation roadmaps.
- Develop and maintain an integrated transition schedule.
- Coordinate key IMIT activities across business, operational, technical, and workforce transition streams.
- Track milestones, deliverables, and critical path dependencies.
- Develop and maintain a RAID Log.
- Facilitate risk identification and mitigation planning.
- Track issues, dependencies, decisions, and escalation requirements.
- Provide proactive recommendations to leadership regarding emerging risks and impacts.
- Support meeting planning, facilitation, documentation, and follow-up.
- Establish and maintain action and decision tracking mechanisms.
- Prepare regular status reports, dashboards, and executive-ready summaries.
- Monitor project progress and recommend corrective actions as required.
- Support coordination of transition readiness activities across IMIT teams.
- Assist with stakeholder communications and engagement planning.
- Support organizational readiness activities related to staff transitions, onboarding, offboarding, identity and access management, and associated digital enablement requirements.
