Summary Of Duties

The Nurse Practitioner (NP) is an advanced practice role designed to contribute to the quality of patient care through practice, research and education. The NP promotes wellness and optimal health promotion and provides exemplary quality care through the application of advanced clinical and theoretical knowledge and skills. The NP practices in a manner that reflects the Mission, Vision and Values of Sunnybrook and in compliance with the professional standards of nursing practice as defined by the hospital and College of Nurses of Ontario.

The role will be engaged in coordinating the clinical management of patients with new, chronic or advanced heart failure in the outpatient area. This will include short term medical optimization and long term follow up for patients requiring close monitoring, management and evaluation.

The Nurse Practitioner will collaborate with patients, family members and other health care providers in the provision of safe high-quality patient centred care.

As an advanced practice nurse, the NP will also collaborate with the Patient Care Manager, Clinical Educator and Advanced Practice Nurse to enhance nursing practice through the development, implementation and evaluation of best practices.

The NP will inform the development of key clinical pathways to improve the quality of care, reduce risks, increase patient satisfaction and increase the efficiency in the use of resources.

Lastly, the NP will evaluate patient outcomes through the development and/or use of appropriate metrics that would monitor the program’s quality and best practice

Qualifications/Skills

  • Current College of Nurses of Ontario Certificate of Competence;
  • Master’s degree with Nurse Practitioner Certification;
  • Demonstrated clinical expertise in cardiac surgery or cardiology preferred;
  • Demonstrated clinical expertise in geriatric and chronic conditions;
  • Current B.C.L.S.;
  • Membership in R.N.A.O.;
  • Previous experience as a Nurse Practitioner required
  • Program planning and evaluation skills;
  • Demonstrated professional leadership and ability to be innovative and creative;
  • Excellent interpersonal and communication skills.
  • Expertise in conflict management
