Reporting to the Community Team Manager, the Intake Coordinator is responsible for receiving referrals and requests for service or consultations, completes screening of referrals, schedules psychiatric assessments and triages to appropriate services for the Mood Disorders Research and Treatment Team and Regional Specialty Mental Health programs. The Intake Coordinator liaises with community–based clinicians, physicians, nurse practitioners and regional partners to optimize referral pathways and coordinate timely care.

Skills And Qualifications

  • Social Service Worker Diploma and a current registration with the College of Social Workers and Social Service Workers of Ontario
  • At least two years, progressively responsible, current clinical experience working with people with complex mental health needs, preferred

Position Responsibilities

  • Serves as the first point of contact for individuals and providers seeking access to the program’s services, managing referrals and consultation requests
  • Receives, reviews, and processes referrals, consultation requests, and inquiries from clients, families, healthcare providers and community agencies
  • Conducts initial intake screening to determine referral appropriateness, eligibility, urgency, and service needs in accordance with program criteria
  • Books initial consultations with care providers
  • In collaboration with the Clinical Director, reviews and triages complex referrals
  • Re-direct referrals to other appropriate services if referral does not fall within the program’s scope
  • Monitors referral status and follows up with referral sources and service providers as required Receives consent for and coordinates internal referrals to the allied health services and/or research within the program
  • Maintains accurate and timely documentation within electronic health records and program databases
  • Tracks referral activity, waitlists, assessment scheduling, and service utilization data
  • Ensures confidentiality and privacy requirements are upheld in accordance with applicable legislation, organizational policies, and professional standards
  • Assists with data collection, reporting, and quality improvement activities related to intake and service access
  • Establishes and maintains effective working relationships with referral sources, local service providers, stakeholders and families
  • Works collaboratively with local service providers and community stakeholders in the provision of services for clients
